PCOS blood test: what to expect.

A PCOS blood test measures androgens, cycle hormones and metabolic markers in one draw. You fast for eight to twelve hours beforehand, ideally collect early in your cycle and early in the day, and it takes about ten minutes. Results usually come back within a few business days.

If you have been told PCOS might explain your irregular cycles, acne or hair changes, this is usually the next step. Here is a practical walk-through of what happens, how to prepare, what it costs, and what the results can and cannot do.

What is measured, and why

  • Testosterone and SHBG, which together give the free androgen index: the blood-test half of "signs of high androgens".
  • LH and FSH, where a relatively high LH is a supporting clue.
  • Fasting glucose and HbA1c, because insulin resistance sits behind many PCOS symptoms and shifts long-term risk, and these are the glycaemic markers the guideline recommends.
  • Lipids, for the heart-health baseline.
  • TSH and prolactin, to rule out thyroid and pituitary causes of irregular cycles.

AMH is not part of the standard KnowLuna PCOS panel. In adults a doctor can request it separately, because a high AMH reflects ovaries carrying many small follicles and can stand in for the ultrasound criterion. It is not used that way in teenagers, and the cut-off is specific to the laboratory assay.

Preparing for collection

Fasting matters. Fasting is needed for insulin, and for glucose if your doctor adds it. Lipids are best fasting too, because triglycerides rise after a meal, so the whole panel is collected fasting. Nothing to eat or drink except water for 8 to 12 hours; book a morning appointment and have breakfast afterwards.
  • Cycle day. If you have cycles, day 2 to 5 is ideal for LH and FSH. If they are very irregular or absent, collect any time and note when your last period was.
  • Time of day. Testosterone peaks in the morning, so an early appointment gives the most interpretable androgen result.
  • Contraception. The combined pill lowers androgens and raises SHBG, which can hide exactly the pattern you are testing for. If you are on it, tell your doctor. Our guide to contraception and hormone tests goes marker by marker.
  • Medicines and supplements. List everything you take. Some affect insulin, lipid or thyroid readings.
  • Water and comfort. Water is allowed and makes the draw easier. Wear a sleeve that rolls up.

On the day

Bring your request (KnowLuna sends yours to your phone) and your ID. The usual route is an accredited NATA collection centre near you, and a home kit or a nurse visit is available if you would rather not attend one. Most centres take walk-ins, and a standard draw is two or three small tubes. Expect about ten minutes in the chair, less if the centre is quiet. You can eat straight afterwards, so bring something with you.

What the results can tell you

Whether your androgens are raised, whether your LH and FSH pattern fits, how your blood sugar and lipids are tracking, and whether the metabolic markers suggest your doctor should look further at insulin resistance. Read alongside your symptoms and cycle history, that lets a doctor apply the diagnostic criteria properly.

What they cannot

Confirm PCOS on their own. PCOS needs two of three features: irregular ovulation, high androgens (on bloods or clinically), and polycystic ovaries on ultrasound (or, in adults, a raised AMH in place of the scan), with other causes excluded. Blood tests cover parts of that, and sometimes an ultrasound is still needed. Normal results do not necessarily rule PCOS out either, especially on the pill. Common questions

Do I need to fast for a PCOS blood test?

Yes, if the panel includes fasting glucose or lipids, and most do. Have nothing but water for 8 to 12 hours, which in practice means an early morning appointment after an ordinary night's sleep. The hormone markers do not need fasting, but there is no harm in collecting them at the same time. Bring breakfast for afterwards.

What day of my cycle should I test for PCOS?

Day 2 to 5, counting from the first day of proper bleeding, gives the cleanest LH and FSH baseline. If your cycles are long, unpredictable or absent, do not wait for a period that may not come: collect any time and tell your doctor when you last bled. The metabolic markers are not cycle dependent, and neither is AMH if your doctor adds it.
